Our center offers the following resources.

Monday thru Thursday 8:30am - 4:30pm / Fridays 8:30am - 12:30pm. Please bring the items listed below with you to your scheduled office visit.
•   Your insurance card/cards.
•   If Worker’s Compensation case, then written pre-authorization is required. Please bring the written pre-authorization with you to streamline the paperwork.
•   All radiology studies (should be no older than the past 12 months) and pertain to your current medical problem. The actual films must include either, a CT scan, MRI or x-rays.
•   Prior medical records, such as consultations from the referring physician, operative reports, EMG and nerve conduction results.
•   The name, address, and phone numbers of both the referring physician and primary care physician.
•   A current and complete list of all medications you are taking.
•   The completed New Patient Packet
•   If needed a family member or friend to take notes/help with questions.

We accept most PPO insurance plans and select HMO insurance plans.

We understand that patients may have difficulties when fulfilling their financial obligations. That is why we work with all of our patients, on an individual basis to offer affordable payment options and a financial plan that they are comfortable with.  Don’t allow financial obstacles prevent you from receiving the care you deserve. Contact our patient liaison to answer any financial concerns or questions.
